HOW YOU AND YOUR INSURER WOULD SHARE THE COSTS
Annual Plan Example:
WE’LL DO ALL THE
WORK FOR YOU
WE’LL DO ALL THE
WORK FOR YOU
❖ We’ll verify your substance use disorder benefits and eligibility for treatment.
❖ We’ll estimate and let you know what, if anything, any cost-share(s) will be (based on the level of care needed, length of stay, and our terms with your plan for services and programs).
❖ Deductibles and co-insurance due are collected after all services/programs are rendered.
*DON’T HAVE INSURANCE? We have a self-pay option for our services and programs. It’s based on our service and program charges and is capped at a set fee (it is not pro-rated over the number of days in treatment). For self-pay stays, we charge most of this fee at admission and the remainder at discharge. Intake Specialists will discuss this option with you. Actual length-of-stay for self-pay stays varies, as they are determined by the level of care needed at evaluation and patient progress in treatment.
